Immunizations
: Philadelphia E&T is committed to ensuring that students' vaccinations are up to date. Students who do not have proof of all required vaccines will not be able to attend school unless an exemption form has been submitted and approved by the Office of Health Services.

There are 3 Ways to Comply:
1. Schedule your child a well visit to ensure they receive the required vaccinations. Have your student’s doctor provide you with a Certificate of Immunization.
or
2. Obtain a medical certificate signed by your child’s doctor, detailing the plan to complete vaccines if the next dose is not medically appropriate.
or
3. Submit documentation if your child is exempt from any immunization(s) for a medical, religious or philosophical/ strong moral reason.

Contact your child's doctor or a Federally Qualified Health Center. Federally Qualified Health Centers are “safety net” providers such as community health centers. The main purpose of the FQHC Program is to enhance the provision of primary care services in under-served communities. Any Philadelphia resident is eligible for free or low-cost care at the City’s AHS Health Centers.
To schedule an appointment at a health center call (215) 685-2933.

Mandated Health Screenings:

The Pennsylvania Public School Code requires specific health screenings, based on grade level, to all students in Pennsylvania. These are considered screenings and are not to be a substitute for regular checkups with a pediatrician. School nurses perform the following health screenings:

·       Growth Screening (height and weight)

·       Vision Screening

·       Hearing Screening –Grade11

Physical Examinations:
The Pennsylvania Public School Code requires that students upon original entry to 11th grade be given a "comprehensive appraisal" of their health. The physical is to be completed within 4 months prior to the start of the school year. In addition, Section 1402(c) requires the completion of medical questionnaires which are to become part of the student's health record.
